REGISTRATION - GENERAL INFORMATION:
"INDIVIDUAL" TRAINING REGISTRATION



• ALERT! The INDIVIDUAL registration form is for one person and one or more courses. The GROUP registration form is for a single course with more than one person.

• Registration deadline is one week prior to the class start date. To register for a course, please complete the form below and click the submit button. You and your supervisor will receive a confirmation. In addition, a 3 day reminder email will be sent prior to the class.

• For training class schedule and location please refer to specific course locations, dates, and time.

• Please do not contact individual instructors and/or training locations.

• Each registrant must complete his/her own form and use his/her own contact information.

REGISTRATION INCLUDES:

• Class instructor and materials. (Some classes require the student to furnish items like a scientific calculator etc.)

• Hotel accommodations, mileage, and per diem are the responsibility of the registrant.


CANCELLATION POLICY:

• If for some reason you cannot attend the class, you must email Jessie Seagroves at jessie.seagroves@tn.gov or call her at 1.615.253.5151.
