REGISTRATION - GENERAL INFORMATION:
INDIVIDUAL GHSO TRAINING REGISTRATION



• ALERT! The individual registration form is for one person and one or more courses. The group registration form is for a single course with more than one person.

• Registration deadline is two weeks prior to the course start date. To register for a course, please complete the form below and click the submit button.

• For training course start time, end time, and location please refer to specific course locations, dates, and time.

• You and your supervisor will receive an email confirmation within a day. In addition, one week prior to the scheduled class date, another email will be sent to remind you and your supervisor about the upcoming class.

• Please do not contact individual instructors and/or training locations.

REGISTRATION INCLUDES:

• Class instructor and materials. (Some classes require the student to furnish items like a scientific calculator etc.)

• Hotel accommodations, mileage, and per diem are the responsibility of the registrant.


CANCELLATION POLICY:

• If for some reason you cannot attend the class, you must email Jessie Seagroves at Jessie.Hubbs@state.tn.us or call her at 1.615.253.5151.
